Here's how it works:

• Print-out and Complete the form below and return to:

   P.O. Box 5750
   Rockville, Maryland 20855

• Once the following form is received, a Motor Vehicle Administration form will be sent to you by MDHA

• Complete and Return the Motor Vehicle Administration form and your check made payable to MDHA for $30 (a one time license plate fee of $30 and membership in ADHA / MDHA is required by the MVA)

• Your new license plates will be mailed to you and you then return your old plates to the MVA
